We are developing a large-group health plan for our members and their families. Working with an innovative benefits advisory team that builds high-performing plans outside the typical carrier model, we will be able to offer a MAC-sponsored ERISA plan that provides significant savings and less cost-sharing than insurance plans in the individual or small group markets. These plans are built on high-value, concierge primary care, high-touch patient helpline and navigation, and a wide-open network.
If you are interested in having access to the charter year of this health plan, even if you’re not sure until you see the product, please answer a short questionnaire so that we can include you and your family as we build and price the plan. Anyone not included in the early underwriting for the plan may not be able to join in the first year.
Again, filling out the questionnaire does not obligate you to a product you haven’t seen yet, it just allows you the option to consider joining later. The instructions for completing the secure questionnaire are below.

Click on Register for New Account and register as a new user.
Create a username, type in your email (will only be used to communicate about this process), select your company (Michigan Association of Chiropractors), choose a security question and answer, then check "I'm not a robot". Finally, press "Register".
You will be sent a confirmation email from "[email protected]", and simply click on the first link to set your password.
Once your password has been reset, click "Log In" and use your newly created username and password.
Click on "Submit Health Application" in the red box towards the top.
Here you will enter all of your demographic and health information for you and any eligible dependents, then click submit.
You are all done!
Once we meet the threshold of submitted questionnaires and perform the necessary analysis and review, we will be better able to determine the feasibility of the MAC Group Health Plan. Please stay tuned for more details as they become available.
